Registration
Accent Physician Specialists now uses the Phreesia system for patient registration. Approximately three days before your appointment, you will receive an email with a link that will allow you to complete your registration paperwork from the convenience of your cell phone, tablet or computer. If you cannot complete your registration prior to your appointment, we have several tablets that you can use to complete your registration upon arrival for your appointment.
If you are able to complete your paperwork prior to arrival at our offices, please arrive 15 minute ahead of your appointment. If you are unable to complete your paperwork prior to your appointment, please arrive 30 minutes early for your appointment.
Preparing For Your Appointment
Because appointments can be brief, and because there’s often multiple issues, it is best to be well prepared for your appointment. Here’s some information to help you get the most from your appointment.
- Make sure to write down your appointment time. We will call to remind you, but in case we don’t connect, we don’t want you to miss your chance to see the doctor.
- We will require a picture ID and any insurance information. If you would us to bill your insurance, we will need these items to meet our obligation to your insurance company.
- Be aware of any pre-appointment restrictions. At the time you make the appointment, be sure to ask if there's anything you need to do in advance, such as restricting your diet.
- Write down any symptoms you're experiencing, including any that may seem unrelated to the reason for which you scheduled the appointment.
- Write down key personal information, including any major stresses or recent life changes.
- Make a list of all medications, as well as any vitamins or supplements that you're taking.
- Take a family member or friend along, if possible. Sometimes it can be difficult to soak up all the information provided to you during an appointment. Someone who accompanies you may remember something that you missed or forgot.
- Write down questions to ask your doctor.
Some basic questions to ask the doctor include:
- What is likely causing my symptoms or condition?
- Are there other possible causes for my symptoms or condition?
- What kinds of tests do I need?
- Is my condition likely temporary or chronic?
- What is the best course of action?
- What are the alternatives to the primary approach that you're suggesting?
- I have these other health conditions. How can I best manage them together?
- Are there any restrictions that I need to follow?
- Are there any educational materials that I can take home with me?
